Regularly share information, exchange insights, and address any challenges or concerns together.
  2. Embrace a Team Approach: Encourage a team mindset among support workers and providers involved in the participant’s care. Foster a collaborative environment where everyone’s expertise is valued, and each person understands their role in achieving the participant’s goals.
  3. Share Resources and Knowledge: Collaborate by sharing resources, best practices, and valuable insights within the NDIS industry. This can include participating in professional development opportunities, attending workshops, and engaging in peer-to-peer learning networks.
  4. Coordinate Care Plans: Work together to create comprehensive and integrated care plans for participants. Ensure that all support workers and providers are aligned in their understanding of the participant’s needs, goals, and preferences. Regularly review and update care plans as required.
  5. Prioritize Participant Choice and Control: Actively involve the participant in decision-making processes regarding their care.
